This program caters to individuals, families, and service providers looking for Assertive Community Treatment (ACT), care management, and residential or housing support for both adults and children dealing with mental health challenges. It evaluates the strengths and requirements of those referred and formulates a plan to connect them with the most suitable services. Additionally, it collaborates with Health Homes to ensure the best Care Management service is provided when necessary.
Intake Process: Interested individuals should call for information; eligibility and service levels are assessed through an application.
Cost: Free of charge.
