About this resource
The Surrogate Decision Making Committee programs provide support in making medical decisions for individuals with disabilities. This includes assistance with dental procedures, medical care, and end-of-life choices.
Intake Procedure: Referrals must come from the New York State Justice Center for the Protection of People with Special Needs; please visit their website for more information.
Fees: Services are provided at no cost.
Who can use this
- This program, managed by Empowered Pathways in select counties of New York, serves individuals with intellectual, developmental, mental health, or substance abuse disabilities who cannot make their own decisions and lack a suitable surrogate.
