A diverse array of services is available to meet the unique needs of each person, including nursing, home health aides, therapy, medical supplies, adult day social care, non-emergency transportation, nutrition assistance with meal delivery, and more.

Who can use this
- Individuals aged 18 and older can enroll if they qualify for Medicaid and have been assessed by a nurse for the necessary level of care.
