About this resource
This program provides case management support to adults dealing with chronic health issues, serious mental health disorders, or HIV/AIDS, aiding them in maintaining their health and avoiding inpatient care, hospital visits, and emergency room usage.
Intake Procedure: Referrals are welcome from healthcare providers, community organizations, individuals, and family members.
Fees: There are no fees associated with this service.
Who can use this
- Participants must: 1) Have active Medicaid 2) Either have two or more chronic health conditions, be living with HIV/AIDS, or have a serious mental illness.
