About this resource
This program offers case management to adults dealing with chronic health issues, serious mental illness, or HIV/AIDS to help them maintain their health and prevent the need for inpatient care, hospital visits, and emergency room services.

Who can use this
- To qualify, individuals must: 1) Have active Medicaid 2) Experience two or more chronic health conditions, OR be diagnosed with HIV/AIDS, OR have a serious mental illness.
