The programs available include: Adult Day Health Care (ADHC) Centers, which provide a secure and supervised space for veterans to engage in activities and socialize, with professional assistance for rehabilitation; Home-Based Primary Care, delivering primary care, nursing, medication oversight, nutrition, physical rehabilitation, mental health support, social work, and referrals to VA community services for veterans facing challenges in traveling to medical appointments; Skilled Home Care, which offers VA-funded nursing services and therapies like physical, occupational, or speech therapy in the veteran's home for those unable to travel; Homemaker and Home Health Aide Services, providing scheduled assistance with personal care tasks such as eating and bathing; Home Telehealth, utilizing in-home technology for veterans living far from VA Medical Centers to connect with care coordinators; Respite Care, offering temporary relief for caregivers with up to 30 days of care per year in various settings for veterans needing assistance; and Home Hospice Care, which delivers in-home comfort and support for veterans and their caregivers during the advanced stages of terminal illnesses, along with bereavement support for family members. Eligibility requires being a veteran of active military service with an honorable discharge, meeting minimum duty requirements. To apply for health care benefits, veterans must have their discharge papers and complete form 1010EZ, which can be downloaded from https://www.1010ez.med.va.gov/sec/vha/1010ez/ or requested by calling 1-877-222-VETS. Intake involves contacting a VA social worker, case manager, or caregiver support coordinator at the nearest VA Medical Center.
