About this resource
This initiative delivers vital health care and social support to Veteran patients, facilitating their ability to reside at home or with relatives. The services provided encompass therapeutic exercises, recreational activities, health education, medication management, meal provision, dementia care, and assistance for family caregivers.
Intake Procedure: A referral from a physician is required. Veterans can apply either in person or through a mail-in application.

Who can use this
- Must be a Veteran of the US Armed Forces with an honorable discharge; Veterans from allied countries may receive care if authorized.
