About this resource
A collaborative team of healthcare professionals develops tailored care plans for Veterans, offering services such as medication management, wound care, pain management, lab draws, in-home telehealth, and coordination with community care providers.
Intake Procedure: Veterans can apply either in person or via a mail-in application. When applying, veterans must present proof of eligibility as an honorably discharged service member by providing a copy of their DD-214 or military discharge documents.
Fees: Please contact for the most current information, as fees are determined based on income and net worth levels.
Who can use this
- Must be a Veteran of the US Armed Forces with an honorable discharge; Veterans from allied nations may receive treatment if authorized.
