About this resource
This program provides assistance to individuals in areas such as setting goals, promoting health, managing transitions in care, and connecting with community and social services, along with support for patients and families, and advocacy efforts. Participants collaborate with a primary care manager to formulate a tailored care plan.
Intake Procedure: Referrals are made through Health Homes (585-613-7670).
Fees: There are no associated fees.
Who can use this
- To qualify, individuals must have active Medicaid and meet the NYS DOH criteria, which includes having either two chronic physical health conditions or HIV/AIDS with the risk of another chronic condition, or one or more serious mental illnesses, along with significant behavioral, medical, or social risk factors that can be managed through care.
