About this resource
The facility serves as a central hub to support individuals in accessing the services they need, including medical and behavioral health care. This support is provided by the Care Coordination Team, which comprises physicians, clinicians, nurses, dentists, outreach workers, care managers, MSCs, and the individual’s support network.
Intake Procedure: Call to Schedule an Appointment
Who can use this
- Individuals aged 18 and older who are eligible for or currently receive Medicaid and have one of the following: a Mental/Behavioral Health Diagnosis, two chronic medical conditions, or a positive HIV test.
