About this resource
This program provides support to individuals with HIV/AIDS or other chronic illnesses in obtaining housing, medical care, and medication, as well as social services related to food and transportation arrangements.
Intake Procedure: An appointment is necessary; please call for details.
Fees: There are no fees involved.
Who can use this
- To qualify, individuals must: 1) Have active Medicaid 2) Either have two or more chronic health conditions, be living with HIV/AIDS, or have a serious mental illness.
