About this resource
This service aids individuals diagnosed with HIV/AIDS or other chronic illnesses in obtaining housing, medical care, and necessary medications, as well as providing social services related to food and transportation arrangements.
Intake Procedure: An appointment is necessary; please call for details.
Fees: There are no charges for this service.
Who can use this
- To qualify, individuals must: 1) Have active Medicaid 2) Be experiencing two or more chronic health conditions, OR be diagnosed with HIV/AIDS, OR have a serious mental health issue.
