About this resource
This initiative provides integrated supportive housing for individuals with disabilities who are experiencing homelessness. It offers apartments to Medicaid recipients aged 55 and older who need help with daily living activities, as well as individuals aged 18 and above who qualify for services from the NYS Office for People with Developmental Disabilities (OPWDD) and military veterans with disabilities. On-site staff members advocate for residents, help them access community services, offer training in independent living skills, and promote social and recreational activities.
Fees: There are no fees associated with this program.
