About this resource
This initiative supports adults enrolled in Medicaid Managed Care by helping them establish connections with peers and community partners. A Care Manager is responsible for providing continuous assessments and referrals for Adult Behavioral Health Home and Community-Based Services (HCBS). These services cater to individuals aged 21 and older who qualify for the Health and Recovery Plan (HARP) and have behavioral health requirements. Participants can choose to be referred for coordination services to aid in accessing HCBS and other necessities, or the Care Manager may keep track of their HCBS participation.

Cost: There are no fees associated with these services.
Who can use this
- Adults aged 21 and older who qualify for HARP, have Medicaid Managed Care, and live in Chemung, Schuyler, Steuben, and Yates counties.
