About this resource
This initiative facilitates the coordination of services by linking and referring individuals aged 18 to 64 with physical disabilities or those who are 65 and older to appropriate supports, enabling them to live as independently and productively as possible.
Intake Procedure: Managed via the Department of Health Regional Resource Development Center (RRDC).
Fees: Covered by Medicaid, Workers Compensation, or private payment options.

- Individuals aged 18 - 64 with a documented physical disability or those aged 65 and older.
