About this resource
Offers a range of services such as Medicaid Service Coordination (MSC), non-Medicaid Service Coordination, respite care, Community Habilitation, Day Habilitation, and Individual Support Services (ISS) for those with Developmental Disabilities and/or mental health issues, as well as Fiscal Intermediary Services.
Intake Procedure: Clients are encouraged to call the agency for more information.
Fees: Medicaid is accepted.
Who can use this
- Available to youth and adults in Suffolk and Nassau Counties who are eligible for OPWDD and Medicaid; a referral or appointment is necessary, so please call.
