About this resource
This initiative offers a range of home and community-based Medicaid State Plan services, including Other Licensed Practitioner (OPL), Community Psychiatric Support and Treatment (CPST), and Psychosocial Rehabilitation (PSR), designed to enhance the social, emotional, and behavioral well-being of youth aged 5 to 21 in Suffolk County, along with their families or caregivers. The OPL service delivers therapeutic support from a licensed mental health professional, while CPST focuses on assisting clients and their families in improving daily living skills and community relationships. PSR provides practical help and skill application based on therapeutic or CPST services. These services can be utilized individually or in combination.
Intake Procedure: Clients are encouraged to call the agency for more information.
Fees: All services are covered by NYS Medicaid, incurring no additional costs for families.

- Youth aged 5-21 in Suffolk County with Medicaid or Managed Care who face social, emotional, or behavioral health issues.
