Suffolk County Department Health Services Perinatal Health Services Program

This initiative, funded by a NYSDOH grant, seeks to improve the well-being of women between the ages of 12 and 44, along with their families. The Suffolk County MICHC project specifically targets 10 high-risk zip codes in Western Suffolk County: Amityville, 11701; Babylon, 11702; Bay Shore, 11706; Brentwood, 11717; Central Islip, 11722; Commack, 11725; Copiague, 11726; Deer Park, 11729; Lindenhurst, 11757; North Babylon, 11703; West Babylon, 11704; and Wyandanch, 11798. These areas demonstrate significant racial, ethnic, and economic disparities. The Suffolk County MICHC employs various strategies and activities to tackle maternal and infant health issues at multiple levels, focusing on women's health throughout their life stages. The goal is to ensure women maintain optimal health before, during, and after pregnancy. Services include education, case coordination, and home visits for pregnant and parenting women, as well as linking low-income women, teens, and families to health and social services to enhance care utilization and continuity. Additionally, both group and individual education sessions are offered. The MICHC initiative will also establish and manage a collaborative network of local health, social service, and community providers to improve access to services and identify service gaps. The Collaborative aims to recognize and address barriers to health for women of child-bearing age.
