About this resource
Southern Tier Care Coordination, Health Homes delivers care management services to individuals who qualify for Medicaid and have a minimum of two chronic health issues, such as asthma, COPD, diabetes, hepatitis, HIV, mental health challenges, active substance use, or heart disease.
Intake Procedure: You can either call or visit the office to complete a referral. Referrals are also accepted online and can be initiated through your healthcare provider: http://www.carecoordination.org/documents/CommunityReferralForm-HHUNYCentral.pdf

Who can use this
- Individuals aged 18 and older, who are Medicaid eligible and have chronic health conditions.
