About this resource
This program delivers case management and supportive counseling for those diagnosed with chronic diseases. Clients have access to transportation for eligible services, and the program facilitates connections to care and treatment adherence services within a comprehensive HIV primary care Retention Adherence Program (RAP). Medical Case Management works in tandem with these services to ensure clients remain connected to the necessary resources for maintaining their health.
