About this resource
This program delivers a range of home care services specifically designed for Medicaid recipients who are elderly, infirmed, or disabled. Services offered include personal care, case and care management, personal emergency response systems, and long-term home care for those who qualify for nursing home placement but prefer to remain at home. All home care services are prescribed by a physician and overseen by a registered nurse, with care provided by licensed professionals who are contracted and monitored by the Department of Social Services.
