About this resource
This program provides a range of home care services for Medicaid recipients who are elderly, infirmed, or disabled. Services offered include personal care, case and care management, personal emergency response systems, and long-term home care for those who qualify for nursing home placement but prefer to remain at home. All services are ordered by a physician and overseen by a registered nurse, with care delivered by licensed providers contracted and monitored by the Department of Social Services.
