About this resource
OnTrackNY is a pioneering outpatient program aimed at adolescents and young adults who may be dealing with mental health challenges. This initiative supports participants in achieving their aspirations related to education, employment, and personal relationships. Those enrolled will have access to a team that includes a Psychiatrist, Nurse, Therapist, Peer Specialist, and Education and Employment Specialist.

Who can use this
- Eligible participants are individuals aged 16 to 30 who have faced mental health issues for more than a week but less than two years and must be open to collaborating with a diverse healthcare team.
