About this resource
This program provides a variety of support services for those facing intellectual and/or developmental disabilities. Available services encompass case management, personalized support plans for individuals and families, brokerage services for transportation, in-home assistance or residential options, as well as employment and vocational support.
Intake Process: Contact us to obtain an application or arrange an intake appointment.
Location: Accessible via Exit 304 off I-84.
Cost: No fees are associated.
Who can use this
- Individuals with an intellectual or developmental disability who are Oregon residents and U.S. citizens.
Documents you may need
- Proof of citizenship, proof of address, and records of any/all psychological and medical assessments related to the disability.
