About this resource
Delivers a variety of support services for those facing intellectual and/or developmental disabilities. These services may encompass case management, personalized support plans for individuals and families, access to transportation brokerage services, in-home assistance or residential options, as well as employment and vocational training.
Intake Process: Contact to obtain a service application. An appointment will be arranged with an eligibility worker who will assist the applicant throughout the application process.

- Individuals with an intellectual or developmental disability who are Oregon residents and U.S. citizens.
Documents you may need
- Proof of citizenship, proof of address, and records of any/all psychological and medical assessments related to the disability.
