This initiative provides case management and facilitates access to services and resources for individuals eligible due to Intellectual and/or Developmental Disabilities. Service coordinators link individuals to a range of services, which may encompass in-home assistance, residential options, supported living, employment opportunities, alternatives to employment, transportation, protective services, and additional community resources and benefits.

Intake Procedure: To begin, visit the website to fill out the online eligibility referral form. For further assistance with the application or more information, you can call or email. An Eligibility Specialist will conduct an intake interview and guide applicants through the eligibility assessment process.

Travel/Location: The office is situated at the intersection of Kaen and Beavercreek Roads, at the top of the hill in Oregon City, and is accessible via TriMet Bus Line 33, marked for Clackamas Community College/CCC.

Fees: There are no fees associated with this service.

  • Eligibility is for children and adults with Intellectual and/or Developmental Disabilities who reside in Oregon.
