About this resource

This service provides a wide range of primary care options, addressing both acute and chronic health issues, annual check-ups for both children and adults, vaccinations, mental health evaluations, substance abuse assessments, and health education. Additionally, reproductive health services are available, including testing and treatment for STIs/STDs, pregnancy testing, and Pap smears. Support is also offered for applying to Medicaid (OHP) and private insurance via the Federal Health Exchange.

Travel/Location: Accessible via Canby Area Transit (CAT), including the 3X Canby Bus, Route 99X Bus, City Circulator, and the Molalla to Canby Bus.

Fees: We accept OHP (CareOregon, Trillium, Yamhill, PacificSource Marion/Polk County, and Open Card), Medicare, and most private insurance plans. A sliding scale fee is available, and for those without insurance, a $25 down payment is required for the initial visit.

Hours include a break from noon to 1pm on Monday, Thursday, and Friday, and until 6pm on Tuesday and Wednesday.
