This program offers a variety of services aimed at older adults and individuals with disabilities, which include assessments, case management, and the management of SNAP (food stamps), Oregon Supplemental Income Program (OSIP), other Medicaid programs, Medicare Savings Programs, as well as providing information and referrals to coordinated services.
Intake Procedure: Individuals can walk in or call for additional information. Certain programs may require a scheduled in-person or phone interview.
