About this resource
This service offers essential medical care for both adults and children, including routine check-ups, immunizations, management of acute and chronic conditions, prenatal services, substance use evaluations, and health education. Additionally, it aids in the application process for the Oregon Health Plan and private insurance via the Federal Health Exchange.
Intake Procedure: Call to book an appointment.
Travel/Location: Accessible via bus lines 155 and 156, and located near the Clackamas Town Center Transit Center with stop numbers 11453 and 11454. The Max stop Green is at the end of Line 13132.
Fees: Accepts OHP and select private insurance. Offers sliding scale fees for those who are uninsured or underinsured, with a minimum fee of $20 required at the time of the appointment for uninsured patients. Care is provided regardless of the ability to pay.
