- Completed application and any documentation required and applicable: -Current year's federal tax forms -3 months of bank statements -Any federal, state, or local assistance letters, including SNAP and TANF -Annual certification of benefits or Social Security Form 1099S if receiving SSI/SSD benefits -Any support received from family or friends must be supported by a letter from the family member or friend
