About this resource
This service delivers comprehensive medical care for both adults and children, covering general physical examinations, vaccinations, treatment for both acute and chronic conditions, prenatal services, substance use evaluations, and health education. It also aids individuals in applying for the Oregon Health Plan and private insurance options available on the Federal Health Exchange.
Intake Procedure: Schedule an appointment by calling.
Travel/Location: Situated at the intersection of Beavercreek Road and Warner Milne Road, accessible via TriMet Bus Line 33.
Fees: Accepts OHP and various private insurance plans. Sliding scale fees are offered for uninsured and underinsured patients, with a minimum fee of $20 required at the time of the appointment for uninsured individuals. No one is denied care due to financial constraints.
