About this resource
This service delivers essential medical care for adults and children, encompassing general physical examinations, vaccinations, treatment for both acute and chronic illnesses, prenatal services, evaluations for substance use, and health education. Additionally, it helps individuals apply for the Oregon Health Plan and private insurance options available on the Federal Health Exchange.
Intake Procedure: To initiate services, please call to arrange an appointment.
Fees: The service accepts Oregon Health Plan (OHP) and various private insurance plans. For patients who are uninsured or underinsured, a sliding scale fee structure is offered, with a minimum charge of $20 required at the time of the medical appointment for those without insurance. Care is provided regardless of the patient's ability to pay.
