About this resource
Delivers a variety of support services aimed at individuals facing intellectual and/or developmental disabilities. These services can encompass case management, personalized support plans for individuals and families, access to brokerage services for transportation, in-home assistance or residential options, as well as employment and vocational services.
Intake Procedure: To begin, call to obtain an application for Developmental Disabilities services. An eligibility worker will set up an appointment to guide you through the application process.
Fees: There are no charges for these services.
Who can use this
- Individuals with an intellectual or developmental disability who are Oregon residents and U.S. citizens.
Documents you may need
- Proof of citizenship, proof of address, and records of all psychological and medical assessments related to the disability
