About this resource
This program features public health nurses who visit homes to support families with children and youth facing special health care challenges. The home visitors aid families in managing their children's care, linking them to healthcare services and community resources, and ensuring effective collaboration among their child's health care team. Additionally, it assists older youth with special health needs in preparing for the transition into adulthood.

Travel/Location: Services are provided by the Sunset Empire Transportation District.
Fees: There are no costs associated.

- Families with children and youth aged 0-21 who have chronic health or developmental issues.
