About this resource
This program provides a variety of support services for individuals with intellectual and/or developmental disabilities. These services may include case management, personalized support plans for individuals and families, access to transportation brokerage services, in-home assistance or residential options, as well as employment and vocational training.
To initiate the intake process, interested individuals should call to obtain an application for Developmental Disabilities services. An appointment will be arranged with an eligibility worker to guide them through the application procedure.
There are no fees associated with these services.
Who can use this
- Individuals of all ages with an intellectual or developmental disability are eligible, provided they are residents of Oregon and U.S. citizens.
Documents you may need
- Proof of citizenship, proof of address, and records of any/all psychological and medical assessments related to the disability.
