This initiative delivers a variety of services aimed at older adults and individuals with disabilities, such as assessments, case management, administering SNAP (food stamps), the Oregon Supplemental Income Program (OSIP), other Medicaid programs, Medicare Savings Programs, along with providing information and referrals to coordinated services.
Intake Process: You can either walk in or call for further details. Certain programs may require an interview appointment, either in person or over the phone.
