About this resource
This program provides a variety of support services tailored for individuals with intellectual and/or developmental disabilities. These services may include case management, personalized support plans for individuals and families, access to brokerage services for transportation, in-home assistance or residential options, as well as employment and vocational support.
Intake Procedure: Schedule an appointment via phone.
