About this resource
The offerings consist of routine physical exams, sports physicals, treatment for illnesses and injuries, vision and hearing screenings, dental assessments, blood pressure checks, immunizations, mental health services, age-appropriate reproductive health care, routine laboratory tests, prescription medication management, and education on health, fitness, and nutrition.
Intake Procedure: Contact to obtain information or to arrange an appointment.
Fees: Accepts OHP, certain private insurance plans, and provides sliding scale fees. No student will be denied services due to inability to pay.
Who can use this
- Children aged 0-18 and the general OHP public within the school district.
Documents you may need
- Insurance information, if applicable. Income information for those applying for sliding scale fees.
