About this resource
The Oregon Health Plan's Health Related Social Needs program facilitates registration and referrals. Case Managers conduct assessments to identify housing barriers and collaborate with Housing Navigators and external organizations to provide support and financial assistance for rental application fees, deposits, and moving expenses.
