About this resource
This service provides in-home support, encompassing tasks such as housekeeping, personal care, laundry, shopping, transportation, and meal preparation and delivery. It also includes adult day services, which are community-based programs that operate for part of the day without residential facilities. Registered nurse services are available for one-time or periodic assessments, service planning, medication monitoring, provider training, and education. Additionally, case management and options counseling are offered to aid individuals in making informed choices regarding services tailored to their strengths, needs, and preferences. The number of service hours per individual is determined by assessed needs and available funding, typically ranging from 5 to 20 hours monthly, with benefits not subject to estate recovery. For intake, individuals can call or visit the website for additional details, noting that some services may have a waitlist. There are no associated fees.
