This program provides a variety of support services for those with intellectual and/or developmental disabilities. Offerings may include case management, personalized support plans for individuals and families, access to transportation brokerage services, in-home assistance or residential options, as well as employment and vocational support.
Intake Process: Contact us to obtain an application or to arrange an appointment.
Costs: No fees are charged.
