This service offers mental health consultations, evaluations, treatment, and case management for seniors in care facilities or referred by private medical professionals. Enhanced outreach services include intensive case management, counseling, and behavioral interventions.

Fees: We accept OHP and Medicare, with sliding scale fees available.
Who can use this
- Individuals aged 65 and older, as well as those under 65 who are homebound due to a physical disability or ongoing mental health issues.
