About this resource
Young Adults in Transition (YAT) offers a range of support services tailored for young adults dealing with mental health or behavioral difficulties. The YAT Teams work to build connections with young adults, their families, peers, and all relevant support networks. Through transitional treatment and intervention, young adults can successfully graduate from school, pursue further education, secure employment, nurture friendships, and achieve greater independence.

Fees: We accept OHP.
Who can use this
- Young adults aged 14-24 who are connected to the juvenile justice system, currently homeless, or residing in foster care.
