Mosaic Community Health

The organization provides a broad spectrum of primary and preventive health care services to the local community. These services encompass general medical examinations, annual women's health check-ups, STD testing and treatment, pediatric care, immunizations, mental health assessments and counseling, as well as evaluations for substance use. Additionally, it assists individuals with the enrollment and re-enrollment process for OHP.

Intake Procedure: Appointments can be scheduled by phone.

Travel/Location: Located one block east of 27th, with the entrance on NE Conners Avenue. The Orange Bus Route has a stop at the intersection of 27th and Conners Avenue.

Fees: The facility accepts OHP, Medicare, and a majority of private insurance plans. There may be options for sliding scale fees and payment plans.
