This service offers a variety of support options for older adults and individuals with disabilities, such as assessments, case management, management of SNAP (food stamps), the Oregon Supplemental Income Program (OSIP), and other Medicaid initiatives, Medicare Savings Programs, as well as providing information and referrals to coordinated services.
Intake Process: Individuals can either walk in or call for further details. Certain programs may necessitate an in-person or phone interview appointment.
