Mosaic Community Health

The offerings include regular physical examinations, sports physicals, treatment for illnesses and injuries, screenings for vision and hearing, dental evaluations, blood pressure checks, immunizations, mental health support, reproductive health services appropriate for different ages, routine laboratory tests, prescription medication management, as well as education and promotion of health, fitness, and nutrition.

Travel/Location: The School-Based Health Center (SBHC) is situated in a modular unit to the north of the high school building, across the parking area.

Fees: We accept Oregon Health Plan (OHP), select private insurance plans, and provide a sliding scale for fees. No student will be denied service due to an inability to pay.
