About this resource
This program features home visits by public health nurses aimed at supporting families with children and youth who have special health care needs. The home visitors aid families in coordinating care for their children, linking them with healthcare resources and community support, while ensuring collaboration among the child's health team. Additionally, it prepares older youth with special health needs for their transition into adulthood.
