About this resource

This program provides a range of in-home care services such as housekeeping, personal assistance, laundry, shopping, transportation, and meal preparation and delivery. It also includes adult day services, which are community-based programs that offer support for part of the day. Registered nurse services are available for one-time or periodic assessments, service planning, medication monitoring, and provider education and training. Additionally, case management and options counseling are offered to help individuals make informed decisions regarding services based on their unique strengths, needs, and preferences. The number of service hours varies per individual based on assessed needs and funding, typically ranging from 5 to 20 hours each month, and benefits are not subject to estate recovery. For intake, interested individuals should call or visit the website for further information, noting that some services may have a waitlist. There are no associated fees.

Who can use this

  • Adults aged 18 and older who require assistance with personal care tasks, have an income not exceeding 400% of the Federal Poverty Level, possess assets below the average cost of six months in a nursing facility, and reside in their own home are eligible.
