About this resource
Delivers a variety of support services for individuals facing intellectual and/or developmental disabilities. These services encompass case management, personalized support plans for individuals and families, access to brokerage services for transportation, in-home assistance or residential options, as well as employment and vocational training.
Intake Process: Contact to obtain an application for Developmental Disabilities services. An appointment will be arranged with an eligibility worker who will guide you through the application process.
Cost: No fees involved.
Who can use this
- Individuals diagnosed with an intellectual or developmental disability must be residents of Oregon and U.S. citizens.
Documents you may need
- Proof of citizenship, proof of address, and records of any/all psychological and medical assessments related to the disability
