The services offered include routine physical examinations, such as sports physicals; treatment for illnesses and injuries; screenings for vision, dental health, and blood pressure; immunizations; mental health services; reproductive health appropriate for different ages; routine laboratory tests; management of prescription medications; as well as health, fitness, and nutrition education and promotion.
Intake Procedure: Schedule an appointment by calling.
Fees: Accepts OHP, select private insurance plans, and offers a sliding scale for fees. No student will be denied services due to inability to pay.

- Children of school age and facilities located in Grant County.
